Sandra Traconis,APRN has been with C & C Health Care Services since 2024. She graduated with honors in 2021. Sandra works in our Urgent Care and oversees CCHCS medical clinic (weight loss, ED, HRT, and Hair loss) along with telehealth.
The mission of C & C Health Care Services is to enhance the well-being of the community by providing comprehensive health care services including prevention, education, primary care, behavioral health, addiction services, & wound care services with a special emphasis on the military, medically under served, and low income populations. Our goal is to provide a “medical home” for the under served by significantly expanding access to primary care, behavioral health, disease management, prevention, and educational services.
With years of experience, our medical team will assess you and create a custom treatment plan that’s right for you.
